Adding Radio Buttons and Check Boxes to Your Form

Radio buttons are organized in groups and enable the user to choose only one of multiple options. For example, a visitor would have to choose Male or Female from a group of radio buttons labeled Gender.

Check boxes look similar to radio buttons, but they function differently. They enable visitors to give multiple answers to questions. For example, the visitor can indicate the kinds of operating systems he uses, and his response isn't limited to one choice. Instead, he can choose any number of options (Mac, PC, Unix, Other).
