Chapter 31. Have You Done It?

Don’t move to the solution phase until you are sure you have found the root cause. Test your tentative conclusion using the following checklist How to Know When You’ve Found the Root Cause for verification. To ensure you’ve analyzed a problem fully and correctly, check the proposed root cause against questions on the checklist. It must pass all of the tests to be the true root cause. If the results of these evaluations aren’t conclusive, continue working until the tests are passed. The second phase of the PS/DM Outline will break down without the right root cause.
