With this option, Photoshop will alter not only those colors that are not printable, but also the ones that can be reproduced just to make sure the relationship between the colors remains consistent. That way you don't have to worry about one overly vivid color becoming the exact same shade of a less vivid color that's right next to it. The only problem with this feature is that it has no idea which colors are actually in your image, so it shifts colors around even if all the colors in your image are within the range of colors that can be reproduced in CMYK mode. That means that all of the colors in your image will become less saturated just to ensure that colors that can't be printed will get shifted to printable ones.
