In the Navy

Fitness reports and evaluations are regularly generated forms in the Navy. In the past, these forms have routinely carried the Social Security number of the reporting senior officer. In 2004, however, the Navy issued an Administrative Message that permits the reporting senior officer to prepare the individual's copy of the fitness report or evaluation with “000-00-0000” appearing in the block on the form where the reporting senior officer's Social Security number would appear. The original form of the report or evaluation, which is filed with Naval Personnel Command, still will carry the reporting senior officer's Social Security number, but all other copies of the reports and evaluations will not carry the reporting senior officer's Social Security number.
