Disaster Planning: Preparation for Recovery After an Attack

If you know a break-in has been successful, you must take immediate action. First, disconnect your network from the Internet. Then find out what happened.

Your server might have merely been browsed, but if the hacker is lucky and skilled, he or she may have gained Administrator privileges. He or she may have planted a virus or a monitoring program to provide him or her with useful information.
