A1: With Adobe PDF forms you can maintain the look and feel of existing paper forms. Users making the transition from paper to electronic forms will maintain familiarity with the appearance of the forms. Because PDF forms can be viewed and filled out using the free Adobe Reader, they are the perfect option for placing forms on line.
A2: Acrobat provides several options for presenting various choices. Combo boxes provide a drop-down list of choices, from which a user can select one. Radio buttons are used when there are several options, but only one can be selected. For options that allow several choices, use check boxes or list boxes, as these allow for more than one choice. For more information, see “Elements of an Adobe Acrobat PDF Form” in the Complete Acrobat 7.0 Help.
A3: Form data can be extracted manually and saved using the Advanced > Forms > Export Data from Form... command. Using a Submit action, which can be attached to a button, the data can be routed to a script that will send the form information into a database or direct it to an email address.
