Creating Forms

The fastest way to create a form is use a template and customize it to meet your needs. FrontPage’s form templates include a confirmation form and a feedback form, plus a form wizard that walks you through creating a custom form. Of course, you also can build a form from scratch.

To create a form from a template

1.
Choose File > New > Page or Web (Figure 12.1).

When the Task pane appears, click Page Templates (Figure 12.2).

When the Page Templates dialog box appears (Figure 12.3), choose Feedback Form and click OK. When the page appears, save it .

When the Save As dialog box appears, give the form page a distinctive name and title, then click Save (Figure 12.4). You’re now ready to change and add form fields to the template-based form page (Figure 12.5). For details, see Adding Form Fields.
